Social Security
If you need to replace your Social Security card, the fastest way is to register for a My SSA account online. In order to use this method, you will be asked to answer identifying questions, and you will need to:
- Have a valid email address that you can access
- Have a Social Security number
- Have a U.S. mailing address
- Be at least 18 years of age
If the My SSA website does not allow you to order your card, follow these steps:
- Download and complete Form SS-5, Application for a Social Security Card. [link]
- Get a Certified Medical Record (CMR). In order to get one, you will need to be seen by a medical provider who has created a chart for you. A CMR is a printout from a medical office that shows your full name, date of birth, and social security number. In order to be certified, it must be stamped and signed by the provider, and the stamp and signature must be “wet”, meaning they must be original signature (not a copy).
